About Derek
Derek has been with PT Northwest since 2015. He is originally from Nebraska where he grew up being very active on his family’s farm. He has been involved in several sports since he was young and is proud to have had the opportunity to play at the collegiate level.  He was initially introduced to physical therapy through injuries he experienced as a young athlete, and he developed the desire to become a physical therapist after spending time as a student at PT Northwest’s Monmouth clinic. As a Physical Therapist, the most rewarding part of his job is that he gets to work with a multitude of different people and to help them achieve their goals. When he is not at work, he enjoys spending time with his wife and 2 kids, especially with camping, hiking, and swimming. He enjoys reading and is still an avid Corn Huskers fan.

About Jason:
Jason joined PT Northwest in 1993. He currently serves as the Clinic Director at our Springfield clinic and as the Interim Clinic Director in Corvallis. In these roles Jason not only focuses on patient care, but overseeing the day to day operations of these clinics and providing our amazing staff with the resources they need to help our patients reach their therapy goals. Every day Jason’s top priority is making sure patients receive the highest level of care and our staff is put in the best position for success.  When he is not at work Jason enjoys spending time with his wife, Shanda and their two daughters in the outdoors, golfing, and traveling.  A former collegiate athlete himself, Jason is a big Duke University and Pittsburgh Steelers fan.

About Jeff:
Jeff Kundert has been a part of the PT Northwest team since 2004, holding the role of Clinic Director at our South Salem Clinic. Specializing in outpatient orthopedic care, Jeff collaborates with each patient to identify their objectives and develop a tailored program to help them achieve their goals and resume their favorite activities. Outside of work, Jeff enjoys quality time with his wife, Melissa, and their daughter, Olivia. Actively engaged in the community, Jeff volunteers with the Rotary Club. A devoted Oregon State University alumnus, Jeff proudly supports the Beavers, displaying his black and orange colors with enthusiasm. In his leisure time, Jeff appreciates good wine and the art of woodworking.

About Jonathan:
Jonathan joined the PT Northwest family in August of 2021 and is currently serving as the Clinic Director at our Monmouth location. He grew up in a small country town in Pennsylvania at the base of the Appalachian Mountains prior to moving to Michigan for college. He was active in athletics and recreational activity throughout high school and college and got interested in physical therapy due to a love for science and human movement. He is passionate about outpatient orthopedics, loves building positive relationships with his patients, and enjoys the variety of people and cultures that he gets to serve in Monmouth while helping them meet their goals. He enjoys volunteering in the community when he has an opportunity and has been involved with local and out of state free healthcare clinics. When not at work, you will find him spending time with his wife, Xakeila and is active with camping, backpacking, skiing, hiking, and any outdoor activity that the Pacific Northwest offers. He cheers for the Atlanta Braves for baseball and Carolina Panthers for football.

About Josh:
Josh has been a part of the PT Northwest family since 2007. He specializes in orthopedics, including back and neck pain with and without radiculopathies, headaches; post-operative shoulder, knee, and hip rehabilitation; acute and chronic overuse/repetitive strain injuries such as achilles tendinitis, plantar fasciitis, tennis elbow, golfer's elbow, patellofemoral pain, and rotator cuff tendinitis; as well as generalized weakness, deconditioning, and falls prevention. Outside of his professional role, Josh enjoys camping and traveling with his family, cheering on his kids in sports, date nights with his wife, an occasional game of golf, stand-up paddleboarding, river floats, elk hunting, and ongoing household projects. One of Josh's favorite local spots is Urban Grange Coffee in West Salem; he says the smell of freshly brewed coffee and homemade pastries that greets you as you walk in is the best! Josh is also an ASTYM-certified provider, certified in Spinal Manipulative Therapy, a certified HawksGrips Blood Flow Restriction (BFR) Specialist, and has taken extensive continuing education courses on spine and chronic pain care.

About Matt:
Matt VanCampen joined the PT Northwest team in 2003. His current role is as VP of Operations and Compliance for PT Northwest and as the Rehab Manager for Legacy Silverton Medical Center.  As part of his role he is responsible for overseeing inpatient and outpatient services at PT Northwest's Silverton and Woodburn clinic locations. While he says he does miss the day-to-day patient interaction, he enjoys chatting with the patients to hear about how their needs are being met by the staff, which allows him to maximize the value of the services provided to the community. Matt is married to Andrea, who is also a member of the PT Northwest team, and they have three sons that are avid baseball players. They love taking advantage of the Pacific Northwest's outdoor opportunities, including spending time hiking, camping and biking. Matt volunteers at various community activities supported by PT Northwest and Legacy Silverton Medical Center, and helps out as available with youth sports.

About Mike:
Mike Hmura has had several roles since coming to PT Northwest in 1991. He is currently director of the Central Salem Clinic, which focuses on providing physical therapy services to pediatric patients. In addition to serving on PT Northwest's Ownership and Leadership teams, Mike also oversees all clinical internships and works with 17 Doctoral programs four undergraduate programs to find qualified students to train the next generation of physical therapists. Mike and his wife, Trina, have two children and quite the collection of pets. With two dogs, three cats, one hamster, and a bearded dragon named Ducky, there are plenty of things that keep Mike busy at home. He also enjoys woodworking, golfing, and biking in addition to coaching soccer and serving as an Adjunct Professor at Willamette University in Exercise Science.

About Renske:
Renske has been a part of the PT Northwest family since 2008.  Her role for the company has been from staff therapist to the director at our West Salem, Woodburn and now the Silverton location.  She is also part of our leadership team and helps out on the recruiting front.   Renske continues to work with patients, helping them reach their full potential with combining her manual techniques and exercise base knowledge.  Married to Jack since 1999, they have 3 kids that keep them quite busy with sports and travel.  At times she'll convince her kids to help her out in her yard, but they don't really share the same passion for gardening.  Her perfect morning would be a cup of coffee on the back deck and planning out  the day ahead.

About Scot:
Since joining PT Northwest in 1987 after graduation, Scot Campbell has been helping patients achieve their therapy goals. Currently, he works at all of PT Northwest's clinics on an as-needed basis to help fill in when clinics are busy or other therapists are on vacation. He loves working with the rest of the excellent therapists that are part of the PT Northwest team, who still teach him new things all the time. Scot and his wife Kathy have three children. Away from the office, he enjoys working outdoors by doing yard work and woodworking. He also loves hiking, volunteering at church and spending time with his family. He also loves cheering for the Boston Red Sox, Chicago Blackhawks, New England Patriots and the BC Lions.

Stephanie Garcia is the HR Coordinator and Lead PSC at PT Northwest. She holds a Bachelor's degree in Kinesiology from Oregon State University. Outside of her professional role, Stephanie enjoys quality time with her husband, Irvin, and their dog, Banks. She is passionate about soccer, both playing and watching, and loves engaging in card and board games. A coffee enthusiast, she can often be found sipping her favorite brew. One of Stephanie's favorite small businesses in Silverton is Akatsuki, where she highly recommends the Fusion Ignition sushi roll.

About Tiffany
Tiffany Schoonover is a DPT and Clinic Director at our Corvallis location. She graduated from Oregon State University with a Bachelor of Science in Exercise Sports Science and a minor in Chemistry, and she also earned her Doctor of Physical Therapy from George Fox University. She is ATSYM certified and trained in women's health, including pelvic floor physical therapy. In her free time, Tiffany enjoys traveling and exploring the Pacific Northwest, gardening, fishing, hunting, camping – and doing it all with her toddler! Her favorite local restaurant is American Dream, which sells artisan pizza and beer sourced locally in a fun and artistic environment. When asked what her best piece of advice is, Tiffany said to maintain a healthy work/life balance and practice what you preach by staying active and prioritizing self-care.

Prescription: A prescription from your physician is a beneficial piece of information in your initial treatment planning and may be required in order for your insurance to cover your physical therapy. You can start by downloading this prescription form.

In general, Yes. Many insurance companies require you to have a prescription from your physician for rehabilitation services in order for us to provide services to you and receive payment.

Yes. We will make copies of your cards to make sure we are sending bills to the correct address with the correct ID and group numbers.

Your attire will differ based on your diagnosis. If we are treating your back or extremities for example, it’s best to wear clothing that provides easy access to the area such as a pair of shorts, which we have available at the clinic. If you prefer to wear your own shorts, we ask that they have an elastic waistband. If your therapy will require any form of exercise at our on-site gym, please bring athletic shoes, a t-shirt and either shorts or sweatpants.

Each of our clinics welcomes you to schedule appointments based on proximity and convenience. Contact us to make an appointment at the location that’s most convenient for you.

PT Northwest has been recognized as an Outpatient Rehabilitation Facility (ORF) under Medicare since 1997. Unlike Medicare Physical Therapists and Independent Practices, ORF facilities are held to a much higher standard of care. We are required to go through annual reviews by Medicare, including on-site surveys. Other requirements include:

  • Annual review of all patient care policies and procedures by a physician
  • Annual review by an outside company of all therapy equipment
  • Annual review of all clinic locations for accessibility issues by a physically disabled individual
  • Annual review of all infection control and aseptic techniques
  • Annual review and training for handling medical emergencies and natural disasters, such as fire, earthquake, and explosions.
  • Quarterly quality assurance review of a random sample of active and discharged patients
  • Annual statistical evaluation of admissions and discharges into the ORF

We are a contracted provider for all major insurances in Oregon. Our front office staff will try to verify that your insurance will cover your therapy before you come into the clinic. If you still have questions about your insurance coverage, our billing department will also be happy to help you. The billing department can be reached at 503-540-8701. It is the patient’s responsibility to make sure that they are using a contracted provider. Providers are typically listed in the handbook that your insurance company distributes to you or you can find them at the insurance company’s website. If PT Northwest is unable to see you due to insurance reasons, we will do everything we can to give you some alternative options.

All your requirements are the same, but you will also need to bring your parent or legal guardian to your first visit to sign the release forms. You will be required to provide your Social Security number when you schedule your first visit.

PTNW adheres to all the requirements of the Health Insurance Portability and Accountability Act of 1996, commonly known as HIPAA.

All patients are provided notice of Physical Therapy Northwest privacy practices. Patient records are considered protected health information and strict confidentiality is maintained in compliance with HIPAA rules. Records will be released for treatment to referring physicians and for health insurance purposes with the consent of the patient.
