About Amanda:
Amanda joined PT Northwest in 2006 as a Patient Service Coordinator at our West Salem location. Over the years, she has held many other positions including Rehab Aide, Billing Coordinator, Auditor, Trainer, and Billing/Patient Services Manager. Amanda is currently our Director of HR & Administrative Services. In this role she supports and oversees human resources, billing and insurance departments, and patient service coordinator positions by developing and managing processes that promote better patient interactions, efficiencies, claims processing, and employee management. Amanda holds a Bachelor of Science in Business with a focus on Finance and Accounting; she is currently working on her Master’s in Business Administration.   Q&A with Amanda: What’s the most inspiring part of your job? My favorite thing about my job is helping people, both patients and employees. I enjoy seeing patients get physically stronger while watching our employees grow in their knowledge and passion. What’s the best piece of advice you’ve ever been given? Serve people, not the other way around. What are some causes you care about? Volunteering in any capacity whether it be helping a friend or volunteering within a structured organization. Long-term goal is to be part of Marion County Search & Rescue (going to finish my MBA first). If you could posses one super-human power, what would it be and why? Invisibility…playing pranks of people would be awesome!

About Andrea:
Andrea has been a part of the PT Northwest family since 2010. She has been the Lead Therapist for our Legacy Silverton Hospital operations since 2013. She has worked in a variety of settings since becoming a physical therapist. Andrea really enjoys helping patients in their post-operative recovery, and working on early mobilization with patients in the acute care setting. Andrea loves helping people stay as active and independent as they can and appreciates the time that she spends with her patients. Andrea received her Bachelor of Science in General Science with a minor in German in 1994. She graduated with a Masters of Science in Physical Therapy from Pacific University in 1999. Andrea is also a Board Certified Geriatric Specialist. She received this advanced certification from the American Physical Therapy Association. She is a long time West Salem resident. Andrea has three sons who are busy with school, sports, and music. She spends a good portion of her free time at baseball games cheering on her boys. Andrea is also a foodie and loves trying out new places to eat in Salem and cooking for her family. She enjoys being outside, even in the rain, and tries to practice what she preaches by walking, running, and staying active.

Derek has been with PT Northwest since 2015. He is originally from Nebraska where he grew up being very active on his family’s farm. He has been involved in several sports since he was young and is proud to have had the opportunity to play at the collegiate level.  He was initially introduced to physical therapy through injuries he experienced as a young athlete, and he developed the desire to become a physical therapist after spending time as a student at PT Northwest’s Monmouth clinic. As a Physical Therapist, the most rewarding part of his job is that he gets to work with a multitude of different people and to help them achieve their goals. When he is not at work, he enjoys spending time with his wife and 2 kids, especially with camping, hiking, and swimming. He enjoys reading and is still an avid Corn Huskers fan.

About Jason:
Jason joined PT Northwest in 1993. He currently serves as the Clinic Director at our Springfield clinic and as the Interim Clinic Director in Corvallis. In these roles Jason not only focuses on patient care, but overseeing the day to day operations of these clinics and providing our amazing staff with the resources they need to help our patients reach their therapy goals. Every day Jason’s top priority is making sure patients receive the highest level of care and our staff is put in the best position for success.  When he is not at work Jason enjoys spending time with his wife, Shanda and their two daughters in the outdoors, golfing, and traveling.  A former collegiate athlete himself, Jason is a big Duke University and Pittsburgh Steelers fan.

About Jeff:

Jeff Kundert joined PT Northwest in 2004 and currently serves as the Clinic Director of our South Salem Clinic. In his current role, he works with outpatient orthopedic patients. Jeff works with each of his patients to determine their goals and create a program to help them meet those goals so they can get back to doing the regular activities they enjoy.

When he's not at work, Jeff enjoys spending time with his partner, Melissa, and daughter, Olivia. Jeff stays active in the community by volunteering in the Rotary Club and coaching his daughter's basketball team. As a proud alumnus of Oregon State University, Jeff bleeds black and orange and loves to cheer for the Beavers!

He grew up in a small country town in Pennsylvania at the base of the Appalachian Mountains prior to moving to Michigan for college. He was active in athletics and recreational activity throughout high school and college and got interested in physical therapy due to a love for science and human movement. He is passionate about outpatient orthopedics, loves building positive relationships with his patients, and enjoys the variety of people and cultures that he gets to serve in Monmouth while helping them meet their goals. He enjoys volunteering in the community when he has an opportunity and has been involved with local and out of state free healthcare clinics. When not at work, you will find him spending time with his wife, Xakeila and is active with camping, backpacking, skiing, hiking, and any outdoor activity that the Pacific Northwest offers. He cheers for the Atlanta Braves for baseball and Carolina Panthers for football.

About Joshua:
Joshua Christopherson has been part of the PT Northwest family since 2007. As the Clinic Director for our clinic in West Salem, he works directly with patients to see to their physical therapy needs while also overseeing day-to-day operations. Joshua is also part of the PT Northwest Leadership team and serves as co-lead of our Clinical Pillar. Joshua married his college sweetheart, Danielle, in 2005. Together, they have two children that keep them very busy. He has a passion for serving youth of the community, and serves as a wrestling and baseball coach for local youth team. He loves cheering for teams from his alma mater, the Oregon State Beavers, along with the Portland Trailblazers, Chicago Cubs and Miami Dolphins.

About Kennedy:
Kennedy Hawkins, MBA, JD, LLM is now a consultant and advisor to PT Northwest. He continues to be a partner. Prior to January 2023, Kennedy was the President & CEO of PT Northwest for 21 years. He helped PT Northwest become a widely respected physical rehabilitation group at the state and national levels. In 2016, 2017, 2018, 2021, & 2022, PT Northwest was named a Top Oregon Workplace by The Oregonian newspaper. Kennedy was instrumental in helping PT Northwest win the National Association of Rehabilitation Agencies (NARA) 2016 Innovation Award for its use of data analytics. He has 30 years of experience in the physical rehabilitation industry. Kennedy has a very diverse educational background. He received a Bachelor of Science in Political Science from Oregon State University, a Master of Business Administration from Willamette University, a Doctor of Jurisprudence from Willamette University, and a LLM in Taxation from Southern Methodist University. In addition to his PT Northwest responsibilities, Kennedy serves as a NARA Board Member responsible for membership and co-chairs NARA’s Outpatient Special Interest Group. He has been involved with NARA since 1996. During his NARA tenure, Kennedy has served as President, Past President, Vice President, Treasurer, Board Member, and Regional Coordinator. He has won NARA’s George G. Olsen Award and NARA’s Service Award. He has served on numerous industry work groups. Another area of interest for Kennedy is public speaking. He really enjoys connecting with people. Some of his speaking venues have included the American Physical Therapy Association, NARA, Intermountain Healthcare, the Veterans Administration, the US Capitol, and various universities. Kennedy believes strongly giving in back to his community. He has served on numerous nonprofit boards, such as the Willamette Chapter of the American Red Cross. Currently, Kennedy serves on the Advisory Board of Willamette University’s MBA Program as well the Sherwood West Citizen Advisory Committee on Land Use Planning. Kennedy enjoys snowboarding with his family, hiking behind his in-laws' farm in the Highlands of Scotland, international travel, and mentoring teens and young adults.

About Matt:
Matt VanCampen joined the PT Northwest team in 2003. His current role is as VP of Operations and Compliance for PT Northwest and as the Rehab Manager for Legacy Silverton Medical Center.  As part of his role he is responsible for overseeing inpatient and outpatient services at PT Northwest's Silverton and Woodburn clinic locations. While he says he does miss the day-to-day patient interaction, he enjoys chatting with the patients to hear about how their needs are being met by the staff, which allows him to maximize the value of the services provided to the community. Matt is married to Andrea, who is also a member of the PT Northwest team, and they have three sons that are avid baseball players. They love taking advantage of the Pacific Northwest's outdoor opportunities, including spending time hiking, camping and biking. Matt volunteers at various community activities supported by PT Northwest and Legacy Silverton Medical Center, and helps out as available with youth sports.

About Mike:

Mike Hmura has had several roles since coming to PT Northwest in 1991. He is currently director of the Central Salem Clinic, which focuses on providing physical therapy services to pediatric patients. In addition to serving on PT Northwest's Ownership and Leadership teams, Mike also oversees all clinical internships and works with 17 Doctoral programs four undergraduate programs to find qualified students to train the next generation of physical therapists.

Mike and his wife, Trina, have two children and quite the collection of pets. With two dogs, three cats, one hamster, and a bearded dragon named Ducky, there are plenty of things that keep Mike busy at home. He also enjoys woodworking, golfing, and biking in addition to coaching soccer and serving as an Adjunct Professor at Willamette University in Exercise Science.

About Renske:

Renske Rose joined the PT Northwest team in 2008. As the Clinic Director at our Woodburn location, she works directly with patients, and enjoys being a "detective" so she can figure out the root cause of a patient's pain to help them find relief. However, her favorite part about her job is that she gets to socialize with patients and learn from them as she treats them. She believes effective treatment comes from balance between hard work and fun.

Renske has been married for 17 years and has three children, all of whom are very active in sports and activities that keep her busy. She loves spending time outdoors with her family going on ski trips and camping in the summer. She has also spent her time volunteering as a coach for youth soccer and youth basketball teams in the area.

About Shiloh:
Shiloh recently joined PT Northwest in July of 2021 as the Clinic Director at our Corvallis location.  She grew up in a very small mining town in Northeastern New Mexico where she spent the majority of her childhood either playing outside or hanging out at the hospital her mom worked at There she developed a natural love for being active and for wanting to help others.  As a Physical Therapist one of the most inspiring parts of the job she describes is "having the chance to form relationships with so many people who end up changing my life.  So many people think it’s about changing other people's lives, and it is; but what inspires me most is reflecting on how much I learn from patients and how significant some interactions end up being...."  As and individual she describes herself a "Resilient, hopeful, and analytical" with a special talent to always find a great deal.  On any given saturday you can find her either baking or outside enjoying all the pacific northwest has to offer camping and biking with her husband.  Her personal motto is:  "Be a top five. (from the quote that says you’re the sum of the 5 ppl you spend the most time with- I want to be someone other’s want in their top 5)"

Prescription: A prescription from your physician is a beneficial piece of information in your initial treatment planning and may be required in order for your insurance to cover your physical therapy. You can start by downloading this prescription form.

In general, Yes. Many insurance companies require you to have a prescription from your physician for rehabilitation services in order for us to provide services to you and receive payment.

Yes. We will make copies of your cards to make sure we are sending bills to the correct address with the correct ID and group numbers.

Please bring a swimsuit and towel when you attend aquatic therapy at our Northeast Salem location.

Your attire will differ based on your diagnosis. If we are treating your back or extremities for example, it’s best to wear clothing that provides easy access to the area such as a pair of shorts, which we have available at the clinic. If you prefer to wear your own shorts, we ask that they have an elastic waistband. If your therapy will require any form of exercise at our on-site gym, please bring athletic shoes, a t-shirt and either shorts or sweatpants.

Each of our clinics welcome you to schedule appointments based on proximity and convenience. If your doctor has prescribed aquatic therapy, we can treat you at our Northeast Salem clinic. Contact us to make an appointment at the location that’s most convenient for you.

PT Northwest has been recognized as an Outpatient Rehabilitation Facility (ORF) under Medicare since 1997. Unlike Medicare Physical Therapists and Independent Practices, ORF facilities are held to a much higher standard of care. We are required to go through annual reviews by Medicare, including on-site surveys. Other requirements include:

  • Annual review of all patient care policies and procedures by a physician
  • Annual review by an outside company of all therapy equipment
  • Annual review of all clinic locations for accessibility issues by a physically disabled individual
  • Annual review of all infection control and aseptic techniques
  • Annual review and training for handling medical emergencies and natural disasters, such as fire, earthquake, and explosions.
  • Quarterly quality assurance review of a random sample of active and discharged patients
  • Annual statistical evaluation of admissions and discharges into the ORF

We are a contracted provider for all major insurances in Oregon. Our front office staff will try to verify that your insurance will cover your therapy before you come into the clinic. If you still have questions about your insurance coverage, our billing department will also be happy to help you. The billing department can be reached at 503-540-8701. It is the patient’s responsibility to make sure that they are using a contracted provider. Providers are typically listed in the handbook that your insurance company distributes to you or you can find them at the insurance company’s website. If PT Northwest is unable to see you due to insurance reasons, we will do everything we can to give you some alternative options.

All your requirements are the same, but you will also need to bring your parent or legal guardian to your first visit to sign the release forms. You will be required to provide your Social Security number when you schedule your first visit.

Rebound Physical Therapy adheres to all the requirements of the Health Insurance Portability and Accountability Act of 1996, commonly known as HIPAA.

All patients are provided notice of Rebound Physical privacy practices. Patient records are considered protected health information and strict confidentiality is maintained in compliance with HIPAA rules. Records will be released for treatment to referring physicians and for health insurance purposes with the consent of the patient.

Position: Physical Therapy Aide
Position Status: Part Time
Location: Corvallis

PT Northwest was Founded in 1977 and is a locally owned, nationally recognized leader in physical rehabilitation with an immediate opportunity for a part-time Rehabilitation Aide.

Minimum Qualification:
The applicant needs to be a high school graduate and above eighteen years of age.

Primary Responsibilities:

  • Make the patients comfortable and happy;
  • Assist the therapists in rehabilitation.
  • Rehabilitation Aide Responsibilities:
  • Bring patients from the reception area to a room & place chart indoor file container;
  • Clean the rooms immediately after use & clean gym equipment;
  • Assist with modalities after requested by a therapist; • Assist therapist with treatments when properly trained & certified;
  • Wash, dry, and fold laundry when not busy;
  • Adequately stock the rooms with linens (towels and gowns) and supplies (Cavicide/lotions);
  • Occasionally answer the phone, schedule appointments, or make phone calls;
  • Assist with any front desk duties when asked (filing, pulling charts, Q& A’s);
  • Assist with other projects needed in the clinic; and
  • Perform any other duties as requested by director or therapy Miscellaneous Responsibilities: (depending on facility)
  • Assist in closing procedures (cleaning rooms, vacuum, turn off machines); and
  • Secure the building when exiting the clinic

Position:  Patient Services Coordinator

Minimum Qualifications:
The applicant needs to be a high school graduate and above eighteen years of age.

Responsibilities:

  • Make the patients feel comfortable and happy
  • Assists in clerical and secretarial functions
  • Welcome and assist patients
  • Answer phones and make calls
  • Manage new patient referrals
  • Schedule new and existing patients
  • Verify insurance benefits and obtain insurance pre-authorizations
  • Collect office visit co-payments and bill payment
  • Ensure paperwork is complete for new and existing patients
  • Conduct chart reviews to ensure charts are complete and patients are scheduled properly
  • Enter patient information and clinical information into the computer
  • Manage clinical issues lists
  • Manage and distribute requests for medical records
  • Conduct mailings and sort mail
  • Address any concerns from patients
  • Assist in closing duties
  • Perform any other duties requested by the management
