Founded in 1977, PT Northwest is a locally owned, nationally recognized leader in physical rehabilitation. PT Northwest was the first group chosen nationally to utilize Intermountain Healthcare’s Rehabilitation Outcomes Management System to collect, analyze, and report on patient clinical data, in order to improve care and lower costs. Intermountain Healthcare is consistently rated as one of the top four health care organizations in America. What does this mean for our patients? It means our patients on average will get better faster, with less out of pocket cost over the course of treatment compared to other physical rehabilitation groups.

Our patients have enthusiastically responded to our passion for physical rehabilitation. PT Northwest uses the Net Promoter Score (NPS) to measure patient satisfaction. The NPS is widely used by companies such as Amazon, Apple, Nordstrum & Costco. Net Promoter Score ranges from -100 to 100. Currently we average between 75-79 rating by our patients. Per NPS scoring break down, that is a world class ranking.

As a progressive Medicare Certified Outpatient Rehabilitation Facility, our group is held to a higher standard of rehabilitative care when compared to regular physical or occupational therapists in independent practice. Our physical, occupational, and speech therapy services are offered through our clinics located in Oregon’s Willamette Valley, in addition to the area hospitals that we serve.

While growing up, I was constantly active in many different sports to fill most of my free time. Fortunately, after high school I was able to continue my sports career by playing football at Western Oregon University where I eventually worked my way to a starting position on the offensive line. When deciding what to focus on in undergraduate school I knew it would involve my interest in natural science, specifically human biology, which was influenced by my extensive athletic background. I was drawn to physical therapy because I could continue to learn about human biology while also applying that knowledge to improve the lives of those in the community that I am working in. Before beginning my physical therapy program I had an opportunity to work for a non-profit organization that provided work experience for individuals with mental disabilities. Then, in my physical therapy studies I learned an eclectic approach utilizing manual therapy techniques and therapeutic exercise to facilitate independent maintenance for patients with orthopedic injuries. Those combined experiences are why I chose to practice within the general patient population and help a wider range of individuals. Recreationally, I enjoy staying active through running, hiking, or fishing and hunting when time allows. My wife and I enjoy cooking together, traveling, and searching for the best local coffee shops.

He currently serves as the Clinic Director for our clinic in Corvallis. In that role, Jason not only focuses on patient care, but overseeing day-to-day operations at our clinic and providing our amazing staff with the resources they need to help patient reach their therapy goals. Every day, Jason's top priority is making sure patient's receive the highest level of care for and that our staff is put in the best position for success. Jason and his wife, Shanda, have two daughters that are active in sports, which keeps them busy. He likes spending time outdoors with his family, including going water skiing and camping. He also has spend several years coaching basketball and is active in both Rotary and the local Chamber of Commerce. Jason is a big fan of the Pittsburgh Steelers and sports teams from Duke University.

In his current role, he works with outpatient orthopedic patients. Jeff works with each of his patients to determine their goals and create a program to help them meet those goals so they can get back to doing the regular activities they enjoy. When he's not at work, Jeff enjoys spending time with his partner, Melissa, and daughter, Olivia. Jeff stays active in the community by volunteering in the Rotary Club and coaching his daughter's basketball team. As a proud alumnus of Oregon State University, Jeff bleeds black and orange and loves to cheer for the Beavers!

He has a passion for working with patients and creating a program tailored to fit their needs. He likes to challenge patients to work hard to reach their goals so they can return to what they love to do, and believes in creating a fun and inviting atmosphere so they enjoy their therapy sessions. Josh and his wife Leslie live in Corvallis with their four children. As a graduate of Oregon State University and a Corvallis resident, Josh is an avid Beavers fan and cheers for them in all sports. Josh also volunteers his time. He serves as an elder in his church, is an active Rotary member and works with the Monmouth-Independence Chamber of Commerce while also volunteering at his children's school.

As the Clinic Director for our clinic in West Salem, he works directly with patients to see to their physical therapy needs while also overseeing day-to-day operations. Joshua is also part of the PT Northwest Leadership team and serves as co-lead of our Clinical Pillar. Joshua married his college sweetheart, Danielle, in 2005. Together, they have two children that keep them very busy. He has a passion for serving youth of the community, and serves as a wrestling and baseball coach for local youth team. He loves cheering for teams from his alma mater, the Oregon State Beavers, along with the Portland Trailblazers, Chicago Cubs and Miami Dolphins.

In his current role, he serves as President of our operation. Kennedy is responsible for overseeing overall operations and continuing to developing a culture that allows us to attract the best staff for treating our patients. Kennedy also loves interacting with patients and listening to their stories about how PT Northwest has transformed their lives. Away from the office, Kennedy loves spending time with his wife, three sons and the rest of his family, whether it's snowboarding or hiking in the Highlands of Scotland. He also loves cheering for the Oregon State Beavers, LA Dodgers, LA Lakers and the Oakland Raiders. Kennedy also spends a lot of time volunteering. He has held several board positions for the National Association of Rehabilitation Agencies and Providers, including President, Past President, Vice President, Treasurer and Regional Coordinator. He is also part of the American Physical Therapy Association Key Stakeholder Work Group. Closer to home, he is active in several local non-profit organizations, including serving as a mentor for teens and young adults at his church.

Education SHRM Certified Professional in Human Resources About Leslie Leslie Chrowl has been part of the PT Northwest team since 2003. As Director of Human Resources and Administrative Services, her primary duties involve employee relations. She handles everything from payroll, insurance enrollment, time off management, hiring and onboarding new employees. She also manages PT Northwest's billing office and will work with patients to settle any issues or answer any questions. She has been in the medical field her entire career, but working with PT Northwest is Leslie's first experience in an outpatient rehab setting. Leslie and her husband are empty nesters who love traveling and spending time with family. She considers herself a cat lover, but hopes to soon welcome a dog into her home since family member allergies keep her from owning a cat.

In his current role as Manager of Rehab Services for Legacy Silverton Medical Center, he is responsible for overseeing inpatient and outpatient services at PT Northwest's Silverton and Woodburn clinic locations. While he says he does miss the day-to-day patient interaction, he enjoys chatting with the patients to hear about how their needs are being met by the staff, which allows him to maximize the value of the services provided to the community. Matt is married to Andrea, who is also a member of the PT Northwest team, and they have three sons that are avid baseball players. They love taking advantage of the Pacific Northwest's outdoor opportunities, including spending time hiking, camping and biking. Matt volunteers at various community activities supported by PT Northwest and Legacy Silverton Medical Center, and helps out as available with youth sports.

He is currently director of the Central Salem Clinic, which focuses on providing physical therapy services to pediatric patients. In addition to serving on PT Northwest's Ownership and Leadership teams, Mike also oversees all clinical internships and works with 17 Doctoral programs four undergraduate programs to find qualified students to train the next generation of physical therapists. Mike and his wife, Trina, have two children and quite the collection of pets. With two dogs, three cats, one hamster and a bearded dragon named Ducky, there are plenty of things that keep Mike busy at home. He also enjoys woodworking, golfing and biking in addition to coaching soccer and serving as an Adjunct Professor at Willamette University in Exercise Science.

As the Clinic Director at our Woodburn location, she works directly with patients, and enjoys being a "detective" so she can figure out the root cause of a patient's pain to help them find relief. However, her favorite part about her job is that she gets to socialize with patients and learn from them as she treats them. She believes effective treatment comes from balance between hard work and fun. Renske has been married for 17 years and has three children, all of whom are very active in sports and activities that keep her busy. She loves spending time outdoors with her family going on ski trips and camping in the summer. This involves him working with directors at each clinic to make sure day-to-day operations runs smoothly for patients. He is also involved in community outreach efforts and marketing our services to patients, and works with strategic partners to build new relationships and develop more business. Tyler and his wife, Melissa, have a 6-year-old daughter and a 3-year-old son who keep them busy, along with a silver lab puppy name Lexi. He spends most of his time with his family. However when he gets the chance he also loves to spend time on the golf course and renovating homes using his DIY skills. Tyler is a big sports fan, and cheers for Texas Tech football, University of North Carolina basketball, and he is an Oregon State Beaver fan when it comes to the in state rivalry! Tyler is a very active community member. He is part of the church board where he attends as well as serving on the Alumni Board of Directors for his alma mater Northwest Nazarene University. Tyler is part of a task force focused on improving healthcare and quality of life for patients and spends a great deal of time working to improve communication pathways between healthcare practices in the Mid-Willamette Valley.

Prescription: A prescription from your physician is a beneficial piece of information in your initial treatment planning and may be required in order for your insurance to cover your physical therapy. You can start by downloading this prescription form.

In general, Yes. Many insurance companies require you to have a prescription from your physician for rehabilitation services in order for us to provide services to you and receive payment.

Yes. We will make copies of your cards to make sure we are sending bills to the correct address with the correct ID and group numbers.

Please bring a swimsuit and towel when you attend aquatic therapy at our Northeast Salem location.

Your attire will differ based on your diagnosis. If we are treating your back or extremities for example, it’s best to wear clothing that provides easy access to the area such as a pair of shorts, which we have available at the clinic. If you prefer to wear your own shorts, we ask that they have an elastic waistband. If your therapy will require any form of exercise at our on-site gym, please bring athletic shoes, a t-shirt and either shorts or sweatpants.

Each of our clinics welcome you to schedule appointments based on proximity and convenience. If your doctor has prescribed aquatic therapy, we can treat you at our Northeast Salem clinic. Contact us to make an appointment at the location that’s most convenient for you.

PT Northwest has been recognized as an Outpatient Rehabilitation Facility (ORF) under Medicare since 1997. Unlike Medicare Physical Therapists and Independent Practices, ORF facilities are held to a much higher standard of care. We are required to go through annual reviews by Medicare, including on-site surveys. Other requirements include:

  • Annual review of all patient care policies and procedures by a physician
  • Annual review by an outside company of all therapy equipment
  • Annual review of all clinic locations for accessibility issues by a physically disabled individual
  • Annual review of all infection control and aseptic techniques
  • Annual review and training for handling medical emergencies and natural disasters, such as fire, earthquake, and explosions.
  • Quarterly quality assurance review of a random sample of active and discharged patients
  • Annual statistical evaluation of admissions and discharges into the ORF

We are a contracted provider for all major insurances in Oregon. Our front office staff will try to verify that your insurance will cover your therapy before you come into the clinic. If you still have questions about your insurance coverage, our billing department will also be happy to help you. The billing department can be reached at 503-540-8701. It is the patient’s responsibility to make sure that they are using a contracted provider. Providers are typically listed in the handbook that your insurance company distributes to you or you can find them at the insurance company’s website. If PT Northwest is unable to see you due to insurance reasons, we will do everything we can to give you some alternative options.

All your requirements are the same, but you will also need to bring your parent or legal guardian to your first visit to sign the release forms. You will be required to provide your Social Security number when you schedule your first visit.

Rebound Physical Therapy adheres to all the requirements of the Health Insurance Portability and Accountability Act of 1996, commonly known as HIPAA.

All patients are provided notice of Rebound Physical privacy practices. Patient records are considered protected health information and strict confidentiality is maintained in compliance with HIPAA rules. Records will be released for treatment to referring physicians and for health insurance purposes with the consent of the patient.
